Once your loved one meets with a social worker, an initial assessment and evaluation is performed to determine his or her need for community resource referrals.
For instance, your loved one may be eligible for a meal program, adult day-care or transportation services.
However, initiating these services can be a time consuming and lengthy process. It requires applications to be filled out and submitted, phone calls with Care Managers, Initial interviews scheduled either at home or at the provider location, among other requirements.
Too often, after the needs are acknowledged, the applicants do not follow through and are left in limbo.
Statewide Homecare makes sure that the gap between the applicant and the service is bridged. With permission from the client, our caregivers offer the service of connecting the applicant with the resource he or she needs by following through on behalf of the applicant to assure that his or her community service is met.
